パスワードを入力しなくてもマシン間で ssh できるようにキーを設定して、しばらくはすべて正常に動作していましたが、突然、一部のマシンでパスワードの入力を求められるようになりました。キーを検証しましたが、すべて問題ないようです。ssh -v を実行したところ、出力は次のようになりました。キーは正常に検証されているようですが、なぜパスワードの入力を求められるのでしょうか?

ちなみに、サーバーが認証を受け入れたというメッセージは表示されません。

~/.ssh/ のモードは 700 です。ssh-copy-id を実行してみましたが、うまくいったようです。念のため、両方のマシンの id_rsa.pub で md5sum を実行しましたが、結果は同じでした。また、ターゲット マシンの authorized_keys のチェックサムは、公開キーのチェックサムと一致しています (これは authorized keys 内の唯一のキーであるため)。

.ssh だけでなく $HOME の権限も重要であることがわかりました。$HOME の権限は 751 以下に設定する必要があります。
