Tienes un utf08 veces en la primera fila pero no has declarado ninguna codificación, por lo que se eliminó silenciosamente, agregué
\usepackage[utf8]{inputenc}\usepackage{textcomp}
Entonces funcionó, entonces la forma más fácil de obtener un encabezado de varias líneas es usarlo \multicolumn{1}para salir Sy luego anidar tabular(probablemente debería haberlo usado, \begin{tabular}[t]pero la imagen muestra la disposición centrada verticalmente por defecto).
