Please do this via either
tlmgr update --self
or by getting the latest updater for Unix-ish systems:
http://mirror.ctan.org/systems/texlive/tlnet/update-tlmgr-latest.sh
and/or Windows systems:
http://mirror.ctan.org/systems/texlive/tlnet/update-tlmgr-latest.exe
Then continue with other updates as usual.

La pregunta es: ¿qué variable PATH usa TexLive-Utility aquí (tenga en cuenta que aparece dos veces en el entorno) y cómo puedo "configurarla" como recomienda TeXLive-Utility, cuando de todos modos crea su propio entorno?

Esto debería ser un problema transitorio y se ha informado a TeX Live en sentido ascendente. Usos de la utilidad TeX Liveactualización-tlmgr-latest.shpara realizar la actualización, debido a problemas históricos con tlmgr update --self.
Como mantenedor de TLU, recomiendo que los usuarios esperen hasta que se solucione en el servidor, pero pueden ejecutar el comando de actualización en la Terminal si están desesperados por alguna actualización del paquete.
Editar: según el equipo de TeX Live, esto ya está solucionado. La versión fija ya debería estar en todos los espejos CTAN.
Es posible que pueda solucionar el problema eligiendo el menú Acción mientras mantiene presionada la tecla Opción y seleccionando el elemento "Instalar TeX Live Manager desde el repositorio crítico". Algunos usuarios informan errores de descarga con este método.
¿Debería marcarse esta pregunta como fuera de tema ahora? Presumiblemente alguien con una puntuación más alta que yo puede hacer eso...
