Recibimos demasiados errores de conexión en condiciones normales, así que supongo que es un problema de configuración. Cuatro servidores web,nousar conexiones persistentes en parte porque eso desencadena el error de demasiadas conexiones demasiado rápido. La máquina está alojada en Amazon. Algunos servidores web no están en la misma zona de disponibilidad que la base de datos. 1a->1d.
Curiosamente, el valor codificado de max_connectionses 400, pero lo establece en 214 al reiniciar. Puedo cambiar ese global del cliente MySQL a 400. Pero ir más allá parece excesivo.

La aplicación es PHP (fcgi/apache).
El tamaño de nuestra base de datos es de aproximadamente 54G. La máquina tiene 36 Gigas de RAM. Free me dice que hay 13G libres, incluso incluyendo toda la RAM utilizada para caché/búferes. Cuando intenté darle 32G a "innodb_buffer_pool_size", MySQL no pudo iniciarse debido a que mmap se quedó sin memoria asignable. El grupo de buffer se encuentra actualmente en 22G.
La salida de mysqltuner mencionó el tamaño del bufferpool y join_buffer_size, que no está en la configuración siguiente.

Problema al depurar mysql con demasiadas conexiones
max_used_connectionses 21 en la salida de MOSTRAR ESTADO.
max_user_connectionsse establece en 0... sin límite.

Tenemos una base de datos separada que usamos para las sesiones que fue configurada para usar conexiones persistentes desde PHP, y de ahí proviene el mensaje de error.
Desactivar conexiones persistentespuede llegar a ser la respuesta después de todo. Lo anterior proviene de no poder encontrar nada inusualmente malo con la configuración de la base de datos principal.
