Login remoto impossível (SSH, interface web), o console efetua logout imediatamente após o login

já há algum tempo que tenho esse problema que (em intervalos irregulares) não consigo mais logar no meu servidor. Uma conexão SSH ou login de interface da web simplesmente não consegue fazer login. O login no console funciona, mas imediatamente após o login eu sou desconectado novamente.

O servidor executa o Ubuntu Server 16.04.2 (LTS) com todas as atualizações mais recentes.

Quando tento fazer login via SSH, recebo o seguinte erro:

$ ssh wouter@Hyperion

Depois de forçar a reinicialização do servidor, tudo funciona novamente por um tempo.

Encontrei o seguinte em /var/log/syslog, que corresponde a uma das minhas tentativas de login malsucedidas:
