Problema ao criar cabeçalho e rodapé específicos

Se não estiver claro, no canto superior esquerdo está a imagem (logotipo da universidade) e no canto superior direito estão os números das páginas.

Temos cabeçalho e rodapé específicos na universidade. Como sou novo em LaTeX, alguém poderia me ajudar a criar isto:

Aqui está uma possibilidade usando obackgroundpacote para colocar a moldura em todas as páginas do documento; a moldura foi construída em dois tabularambientes; alterando os comprimentos \xshe \ysh, (resp.), o usuário pode alterar dinamicamente a separação horizontal e vertical (resp.) entre o texto e o quadro.

simplesmente substitui as imagens reais por retângulos pretos; tornando assim meu código de exemplo compilável para todos. Exclua essa linha do seu documento real e certifique-se de usar o nome correto da imagem do seu próprio logotipo no argumento do \includegraphicscomando.
