Não sei onde realmente surge o seu problema.
Você não deve carregar o color
-package três vezes (duas vezes como color
, uma vez como xcolor
). Eu removi dois deles. Além disso, você MWE não tem um \documentclass
e \begin{document}
.... \end{document}
Eu os adicionei e está tudo bem para mim.

Foi solucionado o erro ao definir uma cor chamada 'gray45', assim:
\definecolor{gray45}{rgb}{0.2, 0.5, 0.478}
Parece que ao inserir um caractere # em uma caixa de listagem, ele amarra para compilá-lo como uma cor, quando não é. Mas, como falei, definindo a cor o erro foi solucionado.
