У меня была похожая проблема, причиной было плохое состояние жесткого диска. Как только я заменил его на новый SSD и выполнил чистую установку Ubuntu, проблема исчезла.

Если это было связано с плохими секторами на жестком диске, я проверил это с помощью инструмента Windows.Жесткий диск Sentinal. Насколько я знаю, плохой сектор не может быть удален каждый раз, иногда это происходит из-за физического повреждения диска. Также моему жесткому диску было 5+ лет, и он достиг своего жизненного времени записи.

На моем новом SSD я сделал чистую установку Ubuntu 21.04, и она работает без сбоев.
