У меня есть такой документ (MWE):

И заголовок индекса отличается от других.

Мне ясно, что я могу как-то использовать idxlayout(возможно, собственный файл стилей). Но я всегда терпел неудачу, когда пытался.

Как указано в разделе 2.3 документации idxlayout, indexпакет должен быть загружен до idxlayout. При правильном порядке загрузки заголовок индекса будет отформатирован как другие (ненумерованные) заголовки секционирования верхнего уровня (в вашем MWE: главы).
